CONSENT FOR RELEASE AND TRANSMISSION OF PERSONAL DATA

1. Consent to Collection, Use and Transmission
I voluntarily consent to the collection, use, storage, and transmission of my personal data provided in connection with my participation in Convention , including courses administered through the IIS online course system.
The personal data covered by this consent may include, as applicable:

    • full name;
    • residential or mailing address;
    • email address;
    • telephone/mobile number;
    • date of birth;
    • course enrollment and attendance information;
    • MCKS courses attended, completed, or enrolled in; and
    • Other information reasonably necessary for the administration and maintenance of my student/course records.

    2. Transmission to Institute for Inner Studies, Inc., Philippines
    I expressly consent to the transmission of the foregoing personal data by Institute for Inner Studies Limited (“IIS Limited”) to Institute for Inner Studies, Inc., Philippines (“IIS Philippines”) for legitimate student and course administration purposes, including:
    A. maintaining and updating student records;
    B. verifying and documenting course participation and attendance;
    C. administering current and future MCKS course registrations;
    D. maintaining records of courses previously attended or completed;
    E. facilitating communication concerning courses, certifications, and related student matters; and
    F. performing other administrative functions reasonably necessary for the proper management of MCKS course records.
    My personal data shall not be used for purposes that are incompatible with the purposes described above without appropriate notice and, where required, my further consent or another lawful basis for processing.

    3. Information Received from Previous or Future Course Organizers
    I also authorize IIS Limited and/or IIS Philippines, as applicable, to receive and process information concerning my participation in MCKS courses from my previous or future instructors and/or authorized course organizers, including my name, contact information, course enrollment, attendance, and course history, where reasonably necessary to maintain accurate and complete student records.

    4. Disclosure to Other Recipients
    IIS Limited and IIS Philippines shall not disclose my personal data to unrelated third parties for purposes inconsistent with this consent.
    However, my personal data may be disclosed to authorized service providers, contractors, professional advisers, governmental or regulatory authorities, or other recipients where such disclosure is necessary for the purposes described in the applicable privacy notice, required by law, or otherwise permitted under applicable data protection laws.
    Where a third party processes personal data on behalf of IIS Limited or IIS Philippines, appropriate contractual, organizational, and technical safeguards shall be implemented as required by applicable law.

    5. International Data Transfers
    I understand that the transmission of my personal data between IIS Limited and IIS Philippines may constitute an international or cross-border transfer of personal data.
    IIS Limited and IIS Philippines shall implement appropriate safeguards for such transfers as required by applicable data protection laws, including the GDPR where applicable and the Philippine Data Privacy Act of 2012 (Republic Act No. 10173), its implementing rules and regulations, and applicable National Privacy Commission issuances.

    6. Voluntary Consent and Withdrawal
    I understand that this consent is voluntary and that I may withdraw my consent at any time with respect to processing that is based on my consent.
    Withdrawal shall take effect prospectively and shall not affect the lawfulness of processing carried out before the withdrawal.
    Withdrawal of consent may also affect IIS’s ability to provide certain services, maintain certain records, or process future course registrations where the processing is necessary for those purposes.

    7. Data Subject Rights
    Subject to applicable law, I understand that I may have rights concerning my personal data, including the right to:

      • request correction of inaccurate or incomplete personal data;
      • request deletion or erasure where legally applicable;
      • object to or request restriction of certain processing;
      • withdraw consent where consent is the legal basis for processing;
      • request data portability where applicable; and
      • Lodge a complaint with the appropriate data protection authority.

      8. Acknowledgment
      I confirm that I have read, understood, and agree to the terms of this Consent for Release and Transmission of Personal Data, as well as the applicable Data Protection and Privacy Policy.
      I confirm that I have been provided with sufficient information regarding the purposes, scope, recipients, and manner of processing of my personal data and that I provide this consent freely and voluntarily.
